Admissions and Records Forms

Some Admissions and Records forms are available in PDF format on their website. However, other commonly used Admissions and Records forms, including "Undergraduate Change of Academic Objective" (used to change or declare major/concentration) "Request for Minor Objective" and "Application for Graduation Check" are currently available as hard copy only. These forms can be obtained at Admissions and Records (LH-114) and may be available in the college department offices and the college advisement center (CP-425).
